Letter 02 · Section 504

Request for 504 Plan Evaluation

Use this letter to request accommodations for a student with a disability that affects a major life activity.

Tip: 504 Plans are for students who need accommodations but don't require specialized instruction. Common for ADHD, anxiety, diabetes, or allergies.

Request for Dual Enrollment Participation

Use this letter to express interest in taking college courses while in high school.

Tip: Dual Enrollment is FREE for DCPS students. Courses count for both high school and college credit.
